A growing number of healthcare organizations are identifying the need to expand the context of their population health initiatives by leveraging alternative sources of data. As an increasing number of data sources are utilized by both providers and payees, these previously untapped data sources are becoming a powerful tool in identifying social determinants of health which play an outsized role in the ability of a patient to achieve improved outcomes over the long term.
