Queensland Health invested $110 Million to roll out their nursing and midwifery navigation tool for 400 senior nurses across Queensland. As the largest integrated care program in Australia, it integrates big data, ICT infrastructure and predictive modeling to determine how we can better coordinate care for a chronic and complex patient cohort. Learn how to:

  • Optimise patient flow and massively reduces length of stay, emergency department representations and preventable hospital readmissions
  • Develop a robust ICT infrastructure, with insight into their “Compass” care coordination system and patient predictive modelling application
  • Use administrative data to help draw a holistic picture of a patient’s clinical profile
