Age Well SG - A Singapore Case Study

Age Well SG is a national programme led by the Ministry of Health, Ministry of National Development and Ministry of Transport to support seniors to age well in their homes and their communities. This theme focuses on translating Age Well SG policy into neighbourhood-level implementation – Active Ageing Centres 2.0, Age Well Neighbourhoods pilots, digital enablement and hospital-community integration for ageing in place.

Redesigning Hospital-to-Community Care

This theme explores how hospitals can enable older adults to safely and effectively transition to care beyond the hospital. Emphasis will be on system-level strategies, including early functional assessment, geriatric principles, and coordination with community nursing, health posts, and home-based services, to create integrated, frailty-ready care pathways.

Managing Frailty Across the Acute Care Continuum

As hospitals face rising volumes of older patients with complex, multi-morbid needs, frailty has emerged as a critical determinant of outcomes across the acute care journey. This theme explores how leading hospitals are moving beyond siloed, unit-based initiatives to implement organisation-wide frailty identification and management strategies that span emergency departments, inpatient wards, and discharge planning.

Bringing Robotics‑Enabled Recovery

As rehabilitation demand rises alongside an ageing population, hospitals and care providers in Asia are increasingly looking beyond traditional, facility-bound therapy models. This theme examines how robotic and exoskeleton-enabled rehabilitation is being deployed not only within hospital settings, but as part of a broader recovery pathway that extends into step-down care and the home.
