Dr Rachel Cheong Chin Yee is a Senior Consultant Geriatrician and Deputy Head of the Department of Geriatric Medicine at Khoo Teck Puat Hospital, Singapore. Trained in geriatric medicine and public health, she focuses on designing and implementing care models that improve transitions for older adults across acute and community settings. She has led the development of specialised short-stay geriatric services and advancing person-centred practices in the acute hospital setting. She is actively involved in clinical research, service redesign, and education, and collaborates with local and international partners to translate evidence into real-world practice. Her work emphasises practical, scalable solutions that enhance patient experience while optimising health system efficiency.
